3. YOUR NAME IN THE CREDITS

Enter your name exactly as you want it to appear in your film. Your screen credit can differ from your legal name. This choice is PERMANENT so choose wisely!

4. PERMISSION TO CREATE YOUR FILM

I confirm that I am the person appearing in the selected footage. I authorize Oliver Warden and Forward Projects LLC to use my recorded image, likeness, and voice, if present, to edit, create, and sell my personalized Quantum film to me, and to make it available for me to watch and download.

This permission applies to my selected film. It does not authorize advertising, promotional use, or sale to other people.

I authorize Oliver Warden and Forward Projects LLC to use excerpts and still images from my personalized Quantum film, including my appearance and recorded voice, if present, to present and promote Quantum on its websites and social media accounts.

This permission does not include my verification selfie, disclosure of my legal name or contact information, or licensing my likeness to third parties for unrelated uses.

6. YOUR PERSONALIZED FILM

A 4K (3840 × 2160) film with opening credits, your selected experience, and end credits with your name. Your completed film will be available to watch and download.
